Service Description

The goal for this appointment is to see if DPC Membership is for you. The Initial Consultation will include meet and greet, goal discussions short term and long term, ensure desired vaccinations are up to date, detailed health history, physical and address any concerns. The fee for this appointment will apply to your one time initial setup fee.
